What Our Palliative Care Service Includes

Our palliative care service provides everything a person needs to be comfortable, well-supported and cared for with dignity at home — including clinical support that works in close partnership with NHS and hospice teams.

Every aspect of a palliative care package is shaped around the individual and their family. Nothing is imposed. Everything is guided by the person's own wishes, their advance care plan and the values and beliefs that matter most to them.

Comprehensive comfort-focused personal care at every stage

Pain and symptom management support alongside district nurses and palliative consultants

Medication administration including syringe driver monitoring (trained carers)

Repositioning and specialist pressure care to prevent discomfort

Nutritional and hydration support according to the individual's own wishes
